NECK & CHEEK SURGERY (RHYTIDECTOMY)

Gradually gravity, environmental factors, personal habits, and hereditary take their toll on our faces.  The skin begins to loosen on the neck and face, forming jowls along the jaw line and a “double chin” effect in the neck, forehead lines become deep creases and the brows may drop, causing a tired look in the eye area.  And, even with the miracles of modern science, aging is still inevitable. 

However, more people than ever before –men and women- are enjoying the benefits that facelift surgery can offer.  A full (neck/cheek, & forehead) facelift or a partial (forehead or neck/cheek) facelift or a mini (mid-face, neck or cheek lift) facelift or the new S-lift, a mini procedure which refines and slightly tightens the jaw line and cheeks (best for a secondary lift or for the younger person) are all available today and offer something to all age groups, usually spanning from the early thirties to the late eighties.  As we all know, we weren’t born with sags in our faces, which can distort our facial shape, give a tired or angry look, and change our view of ourselves.

While a facelift can’t stop aging, it can turn back the clock and give one a more rested, youthful appearance.  In many cases, it truly can make one look as good as one feels.  This is especially important in a competitive job market, where people are working much longer than in the past. 

Many times, Dr. Miles performs an eyelid lift to correct bags and loose skin around the upper or lower eyelids, which contribute to the aging or tired look, and a skin rejuvenation to treat the wrinkles in the skin, usually due to sun damage.  He refers to treating wrinkles, which are usually concentrated in the eye, mouth, and forehead areas, (where we are the thinnest) as the “icing on the cake.”  At times, he also may refine one’s profile with a nasal, chin, or cheek enhancement, or possibly add fat to areas that have been sallow or atrophied from age.  Each case is individualized and planned as a unique work of art.

Facelift incisions are very easily hidden in the hair and around the creases of the ear.  Most facelift patients (even with the more extensive lifts) are able to resume normal (but not strenuous) activities within a week to ten days.  All facelifts are performed under local anesthesia as an out-patient.  Nurses can be arranged for care post surgery as needed.

After a one-on-one in-depth evaluation by Dr. Miles, one can visualize similated changes on the computer video imager before undergoing any surgery.  The imaging session is a marvelous communication tool, where the patient can discuss his or her desires, and the doctor can discuss realistic expectations.  You can view before and after pictures of actual patients, watch a video of Dr. Miles performing facial surgery (if you wish), and receive an itemized fee quote, which outlines all the costs involved.

The patient coordinator will be available to answer any questions that may arise, then or later, and can assist you in planning for your procedure if you wish to do so.

Even after a cosmetic consultation, individuals may need to choose among procedures or adjust the plan to meet their budgets; still others may be apprehensive and need reassurance or have problems with post care, health, etc.

Dr. Miles wants his patients to be fully informed before undertaking any procedure, whether minor or major, and encourages them to come back for a “rediscuss” visit with him and possibly bring a concerned relative or friend along with them.

Moreover, Dr. Miles has a saying, “Your face is my face!”  That means that he will do everything possible to satisfy his patients, which includes listening to their desires and expectations.  There is, however, no “ideal” in a facelift.  Skin type and elasticity, basic bone structure, individual healing, ethnic background, and even previous surgeries have to be taken into account.  The goal should be “improvement” not perfection!  The most important thing is to have good rapport between patient and doctor.

  1. If you come to Dr. William K. Miles at the Advanced Cosmetic Surgery Center, you will receive an in-depth, one-on-one consultation with the Dr. himself (not merely a staff person).  You can discuss your desires, see before and after pictures of actual patients, receive a detailed analysis of your facial structure, problem areas, and suggestions for improvement.  Of course, you must decide what you can afford, what you are  ready to do, and even when you would like to have your procedure/s.  Cosmetic surgery is strictly by choice, and we never “push” anyone into having surgery.  Many times, individuals simply want to pursue  their options and possibly plan for the future.

  2. In addition, Dr. Miles will discuss your health, your medications, your activities, and any other pertinent factors that may affect your surgery.  The procedure itself, the anesthesia, the recovery time, the risks and the benefits will also be addressed.  Dr. Miles believes a patient must be    educated to make the right decision.

  3. Photographs will be taken and reviewed on the video computer imager.  The consultant will create the possible cosmetic changes, which Dr. Miles discussed with you, so you can have an idea what they might look like before undergoing any surgery.  This is a great communication tool for the patient and the Doctor in creating realistic expectations.

  4. You will receive an itemized fee quote, along with information about the Doctor, the facility, and other services our center provides.  This will also help you in planning for your surgery.

SCHEDULING YOUR PROCEDURE

  1. Dr. Miles performs surgery Monday through Friday in his private, fully-accredited surgical facility.  We try to honor your time constraints and schedules, but if you have an exact date that you would like, it is best to schedule early.  You simply call our patient coordinator, tell her a date or time frame you would like, and she will take care of the rest.

  2. Within days, the coordinator will send you a surgery information packet, which details the costs, methods of payment, dates and times of your pre-operative visit and your surgery, pre and post surgical instruction sheets, lab requests, etc.

  3. You will be required to send in a deposit to hold your surgery date.

POST SURGERY

  1. You will remain in our private recovery room until you are stable and ready to go home.  You will be taken by wheelchair (for your safety) to the vehicle and the individual who will be driving you home.  If you do not have someone to care for your postoperatively, we can arrange for a sitter or a nurse.

  2. You will receive oral pain and rest medication as needed.  Usually there is only mild discomfort with facial surgery.  

  3. Elevating the head when lying down and using cold compresses for a few days following surgery will keep swelling to a minimum.

  4. Dr. Miles will see you the following day to remove the initial dressings and any drains that may be needed overnight. 

  5. You will return at the end of one week to have most of your stitches removed, have makeup applied, and in the majority of cases be ready to return to your routine activities.  A few remaining support stitches will remain for another two weeks, but they are hidden from sight.                 

  6. Rigorous activity is to be avoided for about six weeks.  But moderate exercise can be resumed within a few weeks.

Note:  Dr. Miles is always on call for his patients 24 hours a day.

                                TREAT LINES AND SAGS AT THE SAME TIME

More and more cosmetic doctors across the country are discovering the benefits of simultaneous face-lifting with skin rejuvenation – something I have practiced and advocated for twenty years.  In a majority of my cosmetic cases a week, I perform full face skin rejuvenation with complete face-lifting, eyelid surgery, and often profile accents to the cheek, chin, and nose without undue complication.  Combining these multiple procedures saves patients additional operating, anesthesia, and medication costs and time off work and activities.

With facial surgery, I realized early on that a person could recover from two or three facial procedures at about the same rate as one.  And for many people, time and money were the main obstacles to a complete facial rejuvenation.  I wanted them to have the very best result that could be achieved, and consequently, signs of aging are usually evident in more than one area of the face, at least by a certain stage in life.  Also, I noticed that lifting alone did not give a youthful appearance if the surface of the skin looked like a “prune” from sun damage.

Although I performed full face-lifting with skin rejuvenation (then chemical peels)   before there was such a thing as a laser, I found that when lasers were introduced, first for surgery itself, and later, with an attachment for resurfacing the skin as well,  multiple procedure cases became even more effective.  In my hands, I had less bruising and bleeding and discovered a reduction in both the surgical and recovery time.  Moreover, new laser settings and post peel & skincare products made it possible for many patients to look good enough to return to work and activities as early as a week to ten days after surgery. 

I enjoy making people happy and seeing them recapture the beauty that is often hidden behind sags and lines.  Skin rejuvenation to remove the unwanted wrinkles and blemishes is the frosting on the cake to a facelift, avoiding an incomplete makeover look. 

For many years, I have presented at cosmetic surgery meetings, both here and abroad, on the benefits of simultaneous face-lifting and skin rejuvenation, with over twenty years of case results.

 

                               PHYSICIAN DISCOVERS HIS “FIELD OF DREAMS”

After years of soul-searching and a career that has earned him three board certifications and enabled him to glean a broad knowledge of medicine, Dr. William K. Miles has found his “field of dreams.”

While a young medical student at U.T. Southwestern in Dallas, Dr. Miles was assigned to the trauma division at Parkland Hospital, where he had the opportunity to help care for the wounded Lee Harvey Oswald.  Upon graduation, he completed an internship in Internal Medicine at Indianapolis, a residency in General Surgery at Detroit, and another four-year residency in Otolaryngology (head & neck surgery) and Facial Plastic & Reconstructive Surgery at Ann Arbor, Michigan.  At that time, he was being groomed to be the Chair of Head & Neck Surgery at M.D. Anderson and had worked in no less than 28 emergency rooms!  Although he found it gratifying to help others in need, he realized that he would never be able to overcome his sensitivity to a patient’s suffering and in some cases, death.

Fortunately, however, his facial plastic training helped gear him in a different direction – the cosmetic arena, where he has remained for over twenty years.  It was there that he felt he could best benefit others and possibly even transform lives.  “It was and still is,” Dr. Miles explains, “a thrill to affect someone’s life by fulfilling their dreams of an improved appearance, which in turn, boosts their self-esteem and confidence. Cosmetic surgery is about fulfilling dreams, not treating illness.”

Board certified by the American Board of Cosmetic Surgery, which he achieved through years of postgraduate training, case presentations, and a rigorous exam, Dr. Miles is currently serving as an examiner for the Board and trains other physicians both nationally and internationally.  He performs surgery daily at his full-accredited Advanced Cosmetic Surgery Center and has offices in both Dallas and Fort Worth.

STATE-OF-THE-ART COSMETIC CARE WITH A PERSONAL TOUCH

Keeping up with the ever-changing cosmetic surgery field has been a challenge to many practices; however, state-of-the-art care is only as good as the one providing it.  Successful practices today know that patient centered care, stressing personal attention, is what sets them apart. Ask Dr. William K. Miles of the Advanced Cosmetic Surgery Center in Fort Worth. 

Dr. Miles began his practice over twenty years ago when cosmetic surgery was often looked upon as a stepchild to medicine.  “If you weren’t performing reconstructive surgery on a trauma case,  explains Miles, “many people thought the surgery was frivolous.”  This was especially tough on a physician who had trained to do extensive head and neck cancer cases, life-saving tracheotomies & parotidectomies, as well as treat birth defects and every conceivable emergency room case from burns to gunshot wounds.  He traveled a path from internal medicine to general surgery to otolaryngology-- (head & neck) surgery to facial plastic & reconstructive before realizing that cosmetic surgery was his true calling. 

At that time, aesthetic surgery, as we know it today, was actually in its infancy.  There were rather limited facelifts, which did not treat the muscles beneath the skin; there were no endoscopic lifting techniques or high-tech lasers.  And liposuction and modern breast augmentation procedures were still on the horizon.  Certainly, the popularity of cosmetic surgery was yet to come, and few surgeons developed an entire practice around aesthetic procedures alone. 

After years of post-graduate training and a rigorous board examination, Dr. Miles earned his certification by the American Board of Cosmetic Surgery and later became an examiner for the Board.  Utilizing leading edge techniques from the U.S. and abroad, his clientele ranges from school teachers to Hollywood starlets; but they all praise him most for his warmth and genuine concern for his patients.  His commitment to excellence is revealed through his dedication to spending time with his patients, educating them, and helping them to establish realistic goals.  He frequently sees patients late in the evening to accommodate their busy schedules and is always on call for his patients.

Though he has over twenty years of experience and is certified by the American Board of Facial Plastic and Reconstructive Surgery, the American Board of Cosmetic Surgery, and the American Board of ORL/Head & Neck Surgery, Dr. Miles attends workshops and meetings nationally and internationally several times a year to inter-change ideas, to learn new techniques, and to train other physicians.  He serves as an examiner for the Cosmetic Board and spends countless hours in speaking engagements to help educate the public on cosmetic surgery.  “When patients make properly informed choices,” Miles says, “cosmetic surgery can be a very positive experience.”
