FOREHEAD LIFT (BROW LIFT)

Often, the forehead/brow area is the first to show signs of aging, even in the early thirties.  Low brows, deep lines in the forehead, and a tired, angry, or sad expression may not reflect how a person really feels and tends to make one look older as well.  Gravity and the environment (sun  & wind) take their toll on the skin, plus hereditary factors are also involved. 

There are many types of forehead lifts performed today, and Dr. Miles will select the one most appropriate for the individual.  The incisions can be placed at the hairline or behind the hairline through which the forehead skin and tissue is lifted and tightened (trimmed), forehead muscles are treated, brows are suspended, and then sutured into place.  The coronal incision (placed across the crown of the head) raises the hairline slightly.  If someone has a high forehead, the hairline incision is best because it can actually lower the hairline slightly.  With the endoscopic forehead lift, several small incisions are made in the crown and temples to lift the skin and tissue of the forehead by sliding it back, where it is positioned into place and the excess is reabsorbed into the scalp (no excess skin is removed).  It is more difficult to treat the muscles with this approach. 

In addition, there is an upper facelift or subperiosteal forehead lift, where the mid-face (upper cheek) area is lifted as well as the forehead.  With this lift, even down-slanted eyes and/or sallow cheeks can be corrected. 

Recovery time is approximately one week.  This and all facial procedures are performed in our fully-accredited surgical facility as an out-patient.  Discomfort is usually fairly minor and can be controlled easily by oral pain medication.

WHERE TO BEGIN -- THE INITIAL CONSULTATION

If you come to Dr. William K. Miles at the Advanced Cosmetic Surgery Center, you will receive an in-depth, one-on-one consultation with the Dr. himself (not merely a staff person).  You can discuss your desires, see before and after pictures of actual patients, receive a detailed analysis of your facial structure, problem areas, and suggestions for improvement.  Of course, you must decide what you can afford, what you are ready to do, and even when you would like to have your procedure/s.   Cosmetic surgery is strictly by choice, and we never “push” anyone into having surgery.  Many times, individuals simply want to pursue their options and possibly plan for the future.

In addition, Dr. Miles will discuss your health, your medications, your activities, and any other pertinent factors that may affect your surgery.  The procedure itself, the anesthesia, the recovery time, the risks and the benefits will also be addressed.  Dr. Miles believes a patient must be educated to make the right decision.  

 

(4)  

Photographs will be taken and reviewed on the video computer imager.   The consultant will create the possible cosmetic changes, which Dr. Miles discussed with you, so you can have an idea what they might look like before undergoing any surgery.  This is a great communication tool for the patient and the Doctor in creating realistic expectations.

You will receive an itemized fee quote, along with information about the Doctor, the facility, and other services our center provides.  This will also help you in planning for your surgery.   

 

SCHEDULING YOUR PROCEDURE

(1)

Dr. Miles performs surgery Monday through Friday in his private, fully-accredited surgical facility.  We try to honor your time constraints and schedules, but if you have an exact date that you would like, it is best to schedule early.  You simply call our patient coordinator, tell her a date or time frame you would like, and she will take care of the rest.  

 

(2)

Within days, the coordinator will send you a surgery information packet, which details the costs, methods of payment, dates and times of your pre-operative visit and your surgery, pre and post surgical instruction sheets, lab requests, etc.  

 

(3)

You will be required to send in a deposit to hold your surgery date.

POST SURGERY

(1) 

You will remain in our private recovery room until you are stable and ready to go home.  You will be taken by wheelchair (for your safety) to the vehicle and the individual who will be driving you home.  If you do  not have someone to care for your postoperatively, we can arrange for a sitter or a nurse.  

 

(2) 

You will receive oral pain and rest medication as needed.  Usually there is only mild discomfort with facial surgery.  

 

(3)

 Elevating the head when lying down and using cold compresses for a few days following surgery will keep swelling to a minimum.  

 

(4)

 Dr. Miles will see you the following day to remove the initial dressings and any drains that may be needed overnight.   

 

(5)

You will return at the end of one week to have most of your stitches removed, have makeup applied, and in the majority of cases be ready to return to your routine activities.  A few remaining support stitches will remain for another two weeks, but they are hidden from sight.

 

(6) 

Rigorous activity is to be avoided for about six weeks.  But moderate exercise can be resumed within a few weeks.   

Note:  Dr. Miles is always on call for his patients 24 hours a day.

                                 CHANGING THE MOOD OF YOUR UPPER FACE

Invariably, when patients of any age come into my office, complaining that they look angry or tired, it is because of their upper face.  With the continual pull of gravity, or possibly hereditary factors, the brows begin to drop, pushing down on the skin of the upper eyelids and producing a heavy, tired look in the eyes.  Often, the habit of raising the eyebrows to compensate for the heaviness creates deep lines in the forehead or one may squint excessively, resulting in deep frown or worry furrows.

Family, co-workers, and peers may misunderstand one’s true physical or emotional state from this angry or tired appearance.

Forehead lifting, which raises the brows to a more pleasing position, treats the muscle structure beneath the surface and tightens the skin – bringing the upper face to a more rested, youthful appearance. 

Many types of forehead lifts are available today, including endoscopic lifts through small incisions in the hair.  There is the standard lift, which can improve a high or low hairline, and the extended forehead lift, which also gives an upward pull to the cheeks, improving mid-face sallowness and in, some cases, correcting down-slanted eyes.

Often, a laser eyelid lift to treat excess fat and/or skin in the lids themselves is done in conjunction with the forehead lift, as well as skin rejuvenation to treat the surface wrinkles in the skin.  Any or all of these procedures can be performed as an outpatient under local anesthesia.  Most patients recover enough to go out in public in a week.

Let’s face it!  People do judge us by our appearance, especially in the eye area.  They also connect health, energy, and clean living to the condition of one’s upper face, no matter how erroneous that may be in reality.  But perhaps the most important thing is that your tired, sad, or angry expression will not only make you look older than your years, but also does not reflect how you truly fee.
